Acceptable forms of proof of vaccination include:

  • CDC-issued vaccination card
  • Photo of CDC-issued vaccination card
  • A digital copy obtained from the Colorado Immunization Information System, or the myColorado app.
  • You can download the app at mycolorado.state.co.us and once you verify your Colorado ID or Driver License, use it for proof of ID and age as well.
  • If you have an exemption from receiving vaccinations due to medical reasons, you will need to provide a medical exemption signed by a medical professional AND have a negative COVID-19 test result within 72 hours of VinCO.

Please register online prior to testing to ensure our sites are open and appointments are available during the holiday/winter season.  Anyone with an appointment will be notified if their test needs to be rescheduled due to inclement weather.
